Crowley, of Alcester Close, in Stoke, denied an allegation of assaulting, ill-treating or neglecting a child in her care to expose the child to unnecessary suffering or injury on March 17.
The Crown Prosecution Service told a previous hearing that police allegedly saw Crowley lifting the pram in the air and throwing it on the floor in Pasley Street, Stoke.
Police said the baby was found to be covered in cigarette ash and had an extremely wet nappy.
Magistrates had earlier ruled the case was so serious that it should be heard at crown court.
Crowley was released on unconditional bail to appear before a judge on November 1.
A date should then be set for a trial before a jury.
